SEEK Parent Questionnaire
SEEK, Center for Families
The Safe Environment for Every Kid (SEEK) Parent Questionnaire screens for parental depression, parental substance abuse, harsh punishment, major parental stress, intimate partner (or domestic) violence, and food insecurity. SEEK has been validated for use in primary care practice and is intended for parents/caregivers to complete before the child's regular check-up.
Versions of the SEEK Questionnaire are available in English, Spanish, Chinese, Swedish, and Vietnamese.
