As the the second part in the Transforming Complex Care series, following Approaches to Extending Complex Care Models into the Community, this webinar covers best practices in screening social determinants of health, tracking referrals and partnering with community service providers. The focus is complex needs populations and discussing scalable models to identify social needs and track referrals to address social determinants of health (SDOH).
