The RISE National Summit on Social Determinants of Health is the place for cross-sectional thought leaders to discuss actionable, tactical, and scalable solutions to social determinants of health (SDoH) challenges and achieve better outcomes for the most vulnerable populations. Back for its 4th consecutive meeting, this is the only place where stakeholders from across the health care ecosystem convene including health plans, government entities, health care providers, community-based organizations, hospitals, health systems, funders, and pharmaceutical companies.

This year's conference will focus on four key themes: Cross-Sector Collaboration and Making Connections, Use of Data and Emerging Technologies, Funding and Reimbursement for SDoH Initiatives, and Insights into Policy and Legislature and the intersection of the Public / Private sector. With three track themes and four pre-conference intensives, our 2.5 days of jam-packed learning and networking will provide attendees the opportunity to:

  • Learn about recent successes and failures of community efforts to create outcomes-based and sustainable examples of solving for SDoH
  • Take home ideas that you can put to work and avoid ideas that don't work
  • Forge new relationships with other attendees to share and collaborate with those on the front-line solving SDoH
