Webinar: Developing Data Systems for Care Coordination Using Patient-Centered Approaches

Join All In: Data for Community Health for the next webinar in our project showcase series highlighting insights and lessons from local collaborations integrating multi-sector data to improve community health.

This webinar will feature opening remarks from Aaron Sieb, CEO of the National Association for Trusted Exchange (NATE) and National Advisory Board member for Data Across Sectors for Health, followed by presentations from data sharing projects in two communities. Each project is developing a system that integrates data from sectors such as medical, behavioral, social services, and education to improve care coordination for special needs populations. One is working with a health information exchange and the other is building a customized tool.

Presenters will share strategies for developing technical platforms that enable care teams, patients and caregivers to access information that helps them better coordinate services and proactively respond to end-user needs.

Featured projects:
*Children’s Comprehensive Care Clinic has developed a prototype for an application that integrates data from patient EHRs, school records, claims, etc. to improve care coordination for families of children with medical and behavioral complexity.
*Altair Accountable Care Organization is developing an e-Health infrastructure that integrates primary care, behavioral health, and social services to improve the quality of life of people with intellectual and developmental disabilities in the Twin Cities.
