Emerging health system, provider, and payer opportunities in population health delivery

We are living in a time of transition in health care delivery. A paradigm shift has led to an acceleration of value-based programming, further incentivizing care delivery in the right place at the right time. In the near-term, this acceleration poses significant challenges for existing delivery mechanisms in being able to adapt to perform in this changing dynamic. To achieve a population health focus, there must be intentional cooperative behavior, which can lead to a net increase in system value. In this commentary, we will share our view as to how care delivery can become better prepared for the transition to value. This will be viewed through the lens of an integrated model of care, which seeks to align across the continuum to promote health equity, improve care efficiency, and reduce avoidable utilization.
