Join Blue Shield of California and The California Endowment for the second event in our three-part series, Addressing Social Drivers of Health Through Data Innovation and Collaboration. The effective use of data, alongside a strong workforce and sustainable financing, is a critical component to addressing individual social needs and social drivers of health (SDoH). When unlocked, shared, and used effectively, SDoH and social needs data become powerful tools with the potential to aid efforts to improve individual health outcomes, implement population health initiatives, and reduce health inequities through upstream interventions that improve the condition of the communities in which individuals live.

This event will explore how health care and community organizations can work together to improve health outcomes and individual wellbeing through data sharing. This conversation is especially timely with recent policy changes related to the collection and use of SDoH and social needs data presenting new opportunities to leverage data to improve the health of communities.

Objectives for Event:
1. Present a future vision for how the effective collection, sharing, and use of data can improve coordination between health care systems and communities and the ability to address individual social needs and the social drivers of health
2. Build understanding of the opportunities as well as remaining challenges presented by recent state and federal policy changes related to the collection and use of SDoH and social needs data
3. Provide examples of how payers, providers, community-based organizations, and technology companies are leveraging data to address individual social needs and the social drivers of health
4. Identify how stakeholders can strengthen the collection, sharing, and use of SDoH and social needs data to support community-anchored referral networks
