People with complex health and social needs have been disproportionately affected by the COVID-19 pandemic and communities of color continue to face inequitable access to care. At Putting Care at the Center 2021, you'll hear from two keynote speakers, Daniel Dawes and Dr. Angel Love Miles. We’ll learn about the innovative models, tools, and partnerships complex care programs and practitioners across the country are creating to meet their participants’ needs, ensure equity in access to services and care, and promote healing and recovery in their communities.

Registration is open now! Save 30% by registering before August 1. Discounts are available for community health workers, peers, students, and more, and complex care consumers can register for free. Find more information on the registration information page.
