Inaugural National Center for Complex Health and Social Needs Conference held Dec 7-9, 2016

The National Center for Complex Health and Social Needs was launched in late 2016 to serve as a national hub for innovation to better meet the needs of patients with complex health and social needs who are often ill-served by the current system of care. The center’s inaugural conference, Putting Care at the Center, which took place December 7-9, 2016 in Philadelphia, brought together nearly 500 participants including providers, consumers, administrators, researchers and policy experts, to share best practices, network and discuss ways to move the emerging field of complex care forward nationally. The Center is sponsored by the Camden Coalition of Healthcare Providers, which has pioneered integration of medical and social services.
