The Root Cause Coalition’s First Annual Summit on the Social Determinants of Health held Dec 5-6, 2016

More than 400 leaders from healthcare, community-based nonprofits, businesses, the media, government and the faith community gathered on December 5 and 6, 2016 in Chicago for The Root Cause Coalition’s First Annual Summit on the Social Determinants of Health. The event provided a forum for discussion about how the health care and social services sectors can collaborate to address the social determinants of health in order to improve patient and community health and well-being. The Root Cause Coalition is a national, member-driven, nonprofit organization dedicated to addressing the root causes of health disparities by focusing on hunger and other social determinants leading to nationwide epidemic of preventable chronic health conditions.

The 2nd Annual Summit is planned for October 9-10th, 2017 in Louisville, KY.
