Advances in improving social and medical care integration

Hosted in partnership with the UCSF Center for Excellence in Primary Care (CEPC), with support from Blue Shield of California Foundation, this webinar features Laura Gottlieb, MD, MPH, discussing ways to improve social and medical care integration.

Dr. Gottlieb highlights the need for additional evidence regarding programs that address social determinants of health in the health care setting, to better inform program implementation, sustainability, and dissemination. In this presentation, she reviews existing evidence and identifies evidence gaps that will help focus future research.

This webinar is the latest in a series supplementing the Facilitating Care Integration resource center, which offers strategies, tools, and best practices for integrating primary care with various domains of the medical neighborhood.
