AIMS: A care coordination model to improve patient health outcomes

The purpose of this Practice Forum is to provide social work practitioners with information about the ambulatory integration of the medical and social (AIMS) model. AIMS is an innovative care coordination model designed to link patients in primary health care settings with nonmedical supports and services and improve patient self-management and health care utilization. The model was developed at Rush University Medical Center (RUMC) in consultation with a panel of experts in primary care models.
