In the midst of the COVID-19 global pandemic, health care service and delivery have rapidly changed to accommodate new and existing challenges.

Community health centers around the country are adapting to reach their primary care patients and provide services that previously relied on in-person care. We profile two California community health centers that participated in the Population Health Learning Network (PHLN). These organizations are working to ensure that their patients receive the medical and social care necessary to withstand the pandemic and beyond.
