Deploying community health workers to support medically and socially at-risk patients in a pediatric primary care population

We deployed community health workers in an urban, academic pediatric primary care system as part of an approach to family-centered, community-connected population health management. Early learnings highlighted the importance of modified outreach processes, trusted relationships, and alignment with community partners.
