E2 Practice NMDOH: Implementation resources for Non-Medical Drivers of Health

In partnership with the Texas Consortium for the Non-Medical Drivers of Health, SIREN has developed a curated collection of Implementation Resources. The Evidence-to-Practice for Non-Medical Drivers of Health (E2Practice NMDOH)  initiative synthesizes practice-ready resources to support health care systems in implementing screening, referral, and service delivery strategies that address non-medical drivers of health.

The curated collection includes implementation guides, toolkits, evaluation frameworks, and referral protocols, each selected for relevance and practical applicability. Particular attention was given to resources that reflect the opportunities and realities of health care delivery in Texas, while recognizing that many of the included resources are broadly applicable across settings.

All E2 Practice Implementation Resources are included in the SIREN national database, which also includes a wider range of social care evaluation resources. To retrieve only curated implementation materials, users can use the unique search keyword: E2Practice
