Implementing food security screening and referral for older patients in primary care: A resource guide and toolkit
This resource guide from the AARP Foundation and IMPAQ International seeks to address some of the challenges of incorporating food security screening and referrals in primary care settings serving individuals aged 50 and older. The information provided is specific to that population wherever possible; when research is cited that only applies to specific age groups that is noted in the text. Intended for use by health care systems, clinics, and accountable care organizations, the content of the guide synthesizes findings from case studies conducted with health systems that have incorporated food security screening and referral and an environment scan identifying implementation strategies and methods for screening and referral.
