Session Description

This session will highlight emerging implementation science on social screening and interventions in health care settings, and the barriers and facilitators of conducting implementation research on these types of interventions.

Kirsten Bibbins-Domingo, PhD, MD, MAS is the Lee Goldman, MD Endowed Chair in Medicine, Professor and Chair of the Department of Epidemiology and Biostatistics, and Professor of Medicine at the University of California, San Francisco. She is the inaugural Vice Dean for Population Health and Health Equity in the UCSF School of Medicine. She co-founded the UCSF Center for Vulnerable Populations at Zuckerberg San Francisco General Hospital, a research center focused on discovery, implementation, policy, advocacy, and community engagement for communities at risk for poor health and inadequate healthcare. Dr. Bibbins-Domingo is a general internist and cardiovascular epidemiologist who uses observational epidemiology, pragmatic trials, and simulation modeling to examine the impact of clinical and public health approaches to cardiovascular disease prevention in the US, in US population subgroups, and in other countries globally.

Rachel Gold, PhD, MPH, Investigator, Kaiser Permanente Northwest Center for Health Research; Lead Research Scientist, OCHIN

Rachel Gold, PhD, MPH, is an epidemiologist and health services researcher. Her work focuses on the use of health information technology to reduce health disparities, and the implementation methods needed to support adoption of such technologies. She holds a joint position as a Senior Investigator at the Kaiser Permanente NW Center for Health Research and the Lead Research Scientist at OCHIN, Inc. She recently studied how to take an initiative that improved cardiovascular disease and diabetes care in Kaiser Permanente and translate that into community health clinics serving socioeconomically vulnerable patient populations.

Stacy Lindau, MD, MA, Professor, University of Chicago

Stacy Tessler Lindau, MD, MAPP is a tenured associate professor at the University of Chicago and an expert in engineering solutions to injustice together with the patients and communities she serves. Catalyzed by a 2012-15 Health Care Innovation Award from the Centers for Medicare & Medicaid Services, Dr. Lindau is founder, co-owner and Chief Innovation Officer of NowPow, LLC, a social enterprise IT company, and president of MAPSCorps, 501c3, an asset mapping organization that cultivates scientific minds, healthy people and invested citizens from the assets of our communities. Together, these organizations close the information gap about vital community resources while creating meaningful information economy jobs, especially for youth.

Presentation: "CommunityRx: An Asset Based, Community Engaged Approach to Implementing a Community E-prescribing System"

Brigid McCaw, MD, MS, MPH; Medical Director, Family Violence Prevention Program, Kaiser Permanente

Brigid McCaw is the Medical Director of the Family Violence Prevention Program for Kaiser Permanente. She oversees the implementation of a comprehensive, coordinated approach for improving screening, identification, and services for family violence. Dr. McCaw’s leadership, research, and publications focus on developing a health systems response to family violence; ACEs/ trauma and resiliency informed care; and social and behavioral health determinants including abuse/violence. She received her MD from the University of California, San Francisco, and her MS and MPH from the University of California, Berkeley. She is a Fellow of the American College of Physicians, Past President of the National Health Collaborative on Violence and Abuse, and a member of the Forum on Global Violence Prevention, National Academy of Medicine.
