Acess to affordable, equitable healthcare remains a significant health challenge across the nation. Concurrently, the healthcare industry faces the consequences of extensive research spanning decades which has revealed that social determinants of health(e.g., income, education, living and working conditions) profoundly influence health outcomes, often more so than the availability and quality of traditional healthcare services. Healthcare authorities now acknowledge that adverse social circumstances significantly contribute to the nation’s poor health out-comes while healthcare expenditures continue to rise; these two trends are driving a notably swift and substantial increase in healthcare interventions focused on identifying and adressing patients’ health-related social needs within clinical care settings.
