There is evidence to support the association of social determinants of health (such as income, education, and insurance status) on health care and health outcomes.

• Social determinants of health impact transitions of care in complex and often overlapping ways.
• Efforts to mitigate these impacts should be multifactorial and include reducing barriers to health care access and increasing a patient’s social support.
