Accountable health communities: CMS Health-Related Social Needs (HRSN) screening tool

Dr. Billioux and Ms. Verlander share information on the new social needs screening tool developed by Centers for Medicare and Medicaid Services (CMS) for use in the Accountable Health Communities (AHC) model program. Alexander Billioux and Katherine Verlander of the Center for Medicare and Medicaid Innovation at CMS presented on the development and implementation of this recently released 10-item screening tool designed to identify patient needs in the following five domains: housing, food, transportation, utilities, and interpersonal safety. Topics covered in the webinar included how the tool was developed, implementation considerations, and ways the tool will be used in the AHC demonstration.
